By signing you agree all the following program fees are due on the first of each month. The second month will be prorated (unless otherwise agreed by staff) and due on the following first of the month. Late fees will be charged at $10 per day starting the second of each month.

By signing you agree that if the is asked to leave due to failure of compliance, failed UA tests, or any other violation of the signed rules and agreement, all money hereto will be forfieted.
